To ensure the safety of all participants, Night Wolves Wrestling Club requires each athlete (or parent/guardian of a minor) to provide accurate medical information and confirm that they understand the risks associated with training.

You authorize NWWC staff to administer first aid or call emergency services if necessary.

You assume full responsibility for any medical conditions that may affect participation.
